Terms and Conditions of Membership:

Program application only available out our office location at this time. Please be sure to have your bank routing and account number information at time of application. The first month membership is due at time of application.

Membership requires a minimum of 6 or 12-month commitment after which time it remains in effect with a month to month agreement.

Membership discounts do not apply with any other discounts, or to packages or promotions.

We requires 60-days advanced written notice to cancel membership, however you may be liable for remainder of term if cancelled before expiration.

We also understand that from time to time conflict may arise, and we will be glad to accommodate the occasional schedule change. However, should you miss an appointment without any contact, that appointment will not be rescheduled.
